CVSHealth sender transform

This PR adds a sender transform for CVSHealth that makes the following changes per their requirements:

• Suppress OBX's with missing LOINCs
• Convert OBX-5 value to codeable concept using the following value set for SNOMED:
  • Positive = 260373001
    Negative = 260415000
    Invalid = 455371000124106
    Other = 445297001
• Change OBX-2 from ST to CWE to reflect the changes to OBX-5
• Replace "Other" in SPM-4.1 with 445297001 and add SCT for SPM-4.3

Test Steps:

1. download HL7 file from Receive folder for CVS
2. use fhirdata tool with HL7 file and sender transform. For example:
