Made some changes to more closely reflect the original specs, specified Annotation as enum-like where the presence of a certain element reflects the requirement to include annotations for either patient and/or technique.
Changed Anonymize to reflect that its presence denotes that the document should be anonymized (was boolean).
Added the specs for easy reference.
Added known mimeTypes.
