Description of changes

I wrote a plugin inspired by this neat little helper here. My variant does a few more things, eliminating hierarchical issues potentially added by human editors.

Would be lovely if we could link it here.

Fine to also have it here, but plugins can particularly go into https://github.com/rehypejs/rehype/blob/main/doc/plugins.md!
