Stacked on #589. Adds the opt-in governed InvocationDigest lane and a sanitized v2 runtime/dispatch attestation while preserving omitted-input v1 behavior and the regular answer path.\n\nValidation:
node --test npm/tests/unit/governed-codex-profile-contract.exp-0133.test.js npm/tests/unit/governed-codex-runtime-binding.exp-0148.test.js(40/40; zero fail/skip/todo/cancel). Exactly five paths; 86 product and 131 test changed nonblank lines. No model or application-network call.
