Add the counselling task to the family planning follow-up visit for the male

• Counselling is the second task in the family planning follow-up visit. It is a required task for the CHW to record before the visit can be submitted. It always appears in the family planning visit.
• See row 41 in CHW 3 - FP - Follow up visit - Male in the data dictionary

Once task is completed:

• If fp_counselling = "No", task is yellow, otherwise it is green.
• The text under the completed task should be:
