  • Top left X button cancels the home visit and returns the user to the page they were on before (i.e. the family planning client profile page)
  • Before they exit, we need to add a confirmation close message saying that any information they entered will be lost (we can use one of the existing form close confirmation messages from OpenSRP)
  • Submit button remains greyed out until all required questions in the home visit are answered at a minimum.
  • Form title is: "first name middle name last name, age· Follow-up visit". The thing between age and form name is an interpunct or middle dot. A comma is okay if you want to be less fancy.
  • Tapping on the row or the circle next to the service opens the widget for the user to record that service was given (or not)
  • A follow-up visit will only occur for those where: {fp_method_accepted} = "Female condom", "Male condom",
  • A recurring visit will occur for those where: {fp_method_accepted} = "Female condom", "Male condom" according to the service schedule Service Schedule - CHW Tab
  • Add the side-effects task (Add the Side-effects task for male #79 )
  • Add the counseling task (Add the counselling task to the family planning follow-up visit for the male #78 )
  • Add the refill or next health facility task (Add the Resupply or Next facility visit task to the family planning follow up visit for male #77 )
  • Tasks use a similar style to other home visit (i.e. child, ANC, PNC, malaria). Also, shown below
