docs: correct the session log to the merged state - #13

The log still described seven commits sitting on an unmerged branch and claimed the chat from 09f4dda was live. Both were true when written and neither is now: everything went into main via #12, and that chat was a duplicate that did not survive the reconciliation — PR #7 had already landed one on main five days earlier. Records the correction explicitly rather than quietly rewriting it, and points at the handover doc as the maintained list so the two do not drift apart again.
