Should flush more often when indexer is catching up

Currently the indexer does not flush to db if there a very few transactions among large amount of blocks.
Also lcd timeout might cause a error loop if there is high loading between crowded blocks, since indexer will retry from flushed block height, catch up won't have any progress if lcd crash before flush
