Reorganize code and UI

Changes:

• Added copy hyperlink in password reveal modal
• Reordered domain and username texts in account cards
• Changed wordings to various modals
• Added version number in drop down menu

Code changes:

• Moved every modal to their own view and controller files
• Modal are now created on request. Previously, modals are just hidden and shown on demand
