A minor update, errata fixes, a few minor improvements and clarifications. The changes are numerous as I taught with the book this semester and fixed a number of issues, but they are all minor in character.

This version should be backwards compatible with version 5. Mostly just page numbers are changed, but Chapters, sections, exercises are all numbered the same.
