When inserting a dummy variable d, -d must be added to each new subclause, not merely the first one.

bf sometimes create unsatisfiable CNFs from satisfiable formulas, because of a bug with dummy variables. When inserting a dummy variable d, -d must be added to each new clause generated from the sub-conjunction, not merely the first one.
