fix(utils): set default value for failed flag in TaskResult

Description

TaskResult model deserialization fails when the task response returned by a LLM lacks a failure field. By setting default values ​​for the failure field, the program can improve robustness and better accommodate incomplete responses from LLM.

Fixes #3348
