Bug fix in D1a for NORMCOG

Previously, we incorrectly required Section 3 when NORMCOG = 1. This has been fixed and Section 3 is no longer required for normal cognition.

For MOCA delayed recall questions: 'No cue', 'Category cue', and 'Recognition' should have a sum less than or equal to 5.
