Add housing_status field.

I rule and have completed some work on Case Manager that's ready for review!

This pull request makes the following changes:

• Add housing_status field.
• Remove homelessness special circumstance from UI.

• Implements Add housing security status to patient information #3265

This PR needs an accompanying rake task to perform the data migration (Patients with homelessness special circumstance should have housing_status set to Housing insecure).
