Enhanced the UI of review form in review.html

Fixes #199

I have made some changes to the review form..

1. Added placeholders.
2. Increased font size of Close Icon to enhance visibility.
3. Enhanced styling of buttons and inputs.
4. Adjusted the form to ensure web and mobile responsiveness.

Note: Changed the form method to post
