Add common installation issues to the readme

Add e.g.:

Common errors:
ImportError: cannot import name 'InterfaceSurface' from 'vita.modules.projection.cherab'.

Solution: If there are no other errors when importing, this usually means vtk is not installed. Run 'conda install vtk'.
