Independent rendering chapters in html books makes list-of-blocks longer and longer...

As of v0.1.0, list-of-blocks are created by appending references to a file. This collides with quarto updating chapters on the go: the same references are appended over and over.

This issue requires a complete change of implementation of lists-of-blocks

Workaround to obtain "clean" lists-of-blocks: rebuild the book and rename the resulting list-of-xxx.qmd files

Notes to self:

• save crossreferences to some json file: for whole document
• how can we find document name for books?
• at start read crossreferences. at end overwrite listof file
• rewrite concurrently with change of syntax (group names have to be different from class names, otherwise confusion: -> group takes precedence)
