After the Sign In button is clicked, there is no indication that the login form has been submitted. I propose the state of the button changes to a spinner/check like in the Umbraco backoffice (or is disabled) to indicate the click was successful.

With MFA enabled, when the Enter your code page loads the focus is not set to the input box. The user must place the focus/cursor in the input box and then enter the code. I think it would be a nicer experience if the focus was set to the input initially. :-)

I would further the last point by suggesting the default action should be the verify button - so that when the dialog appears you can simply type the verification code and press enter - instead of needing to use the mouse.
