I fixed by changing the line 57:

"for='choice_" . $form['id'] . "" => "for='choice" . $random_id . '_',

I had the same issue, but I had to tweak your code a bit
As of version (2.4.9), I added this and it fixed it

"for='choice_" . $form['id'] . "" => "for='choice_" . $random_id,
