Allow new clubs to apply for "clubs" access level

Currently, we are manually adding new clubs and increasing their access level.

Describe the solution you'd like

Add some place where clubs can apply for an elevated "clubs" access level. This could be a form located somewhere on the site or maybe even on the main sidebar on the home page. Clubs would essentially just apply and give an explanation of their club on the form. Once an admin approves, they are automatically changed to club access level. Due to the fact that clubs can add/edit the club pages, this would allow them to add themselves with only an approval from an admin.

Describe alternatives you've considered

The current method of manually doing it is fine as long as it is clear who are admins (CSO Execs and potentially some other execs) and there is clear communication between execs.
