[Bug]: PLugin is struggling with duplicated in the same note

Describe the bug

If there are duplicated in the same note - system would not recognize the other row and might start adding multiple HTML tags for one line. Next time - when start plugin, the other duplicate entry would still be shown as "new"

To Reproduce

1. Create two duplicate cards in the same note
2. Review cards
3. Observe that one card had double HTML enties, the other one - none.

Expected behavior

Two options:

1. Assign HTML tags to the right card
2. Remove duplicates
