Consider revocation cache uses during a `fetch`

We should ensure that revocation checks are complete and error-free with any possible concurrent fetch operation. Currently the cleaning up of old unused files falls foul of this.

Good point, probably tricky to test well?

Maybe it would work well to move the entire revocation dir at once...
