Visit form - should not be able to select pump options if on MDI #513

Not strictly a bug but feels like one
If the user patient states the user has T1 and is on MDI, they should not then be able to declare what sort of pump system they are using.

This is mostly handled with #572
The options still show (it would be better possible to grey them out) but validation now at least prevents the user from saving a record in the questionnaire if they select options for a patient with T2 when they have T1 and vice versa. In the CSV upload option it is still possible to save these items, but they will be flagged now as errors
