In issues page click on "New Issue".
Your title should have enough info . Avoid title like "Installation Issue". be a bit descriptive. "Installation Error for Ollama on Windows - command not found".
In the description add all necessary files - error logs, steps to follow to reproduce the error, what you tried that didn't work.
Choose appropriate Label from the "Labels" options on the right of new issue page.
