Add the Change or Stop family planning form - male

• From the drop down in the client profile (Implement top right 3 dots menu in profiles #22 ), when a person selects "FP change method", the Change or Stop FP form is launched.
• Form title is "Change or Stop FP form"/"
  Badilisha au acha uzazi wa mpango"
• Follow the exact content from the data dictionary, Tab HF - Change or Stop FP - Male
• Native form widget split into 3 screens: Stop current method (row 4 -11), Family planning method (row 14 - 16), Family planning method info (row 18 - 25)
• When {reason_stop_fp_chw} != "Decided to change method" the record is archived, the man is removed from the family planning register, and the family planning row is removed from his profile
• When {reason_stop_fp_chw} = "Decided to change method", the man's record is updated with the new family planning method chosen, new schedules are calculated, the family planning row is updated

1. On choosing a "Reason the man is stopping her current family planning method" , the next page "Family planning method" has no actionable items but just "Back" and "Submit" buttons
