Implement the refer to facility FAB for males - CHW

Same as previous modules - Refer to #554

• After selecting the "Refer to facility" from profile FAB (#269), it will take you to the screen to select the type of referral #555) based on the client profile unless the client is only eligible for one type of referral. If the client is only eligible for one type of referral, it goes straight to that type of referral
• Question text under the form title should be, "Reason for referral"
• Each row will have a form name based on the type of referral forms the member profile supports (#554)
