This plugin is currently run on our demo server. We have an SLA but it's not presented to people filling in the form. We should add a link or embed the SLA, and require a checkbox to be ticked.

👍 cross-linking to ome/prod-playbooks#197

In particular ome/prod-playbooks#197 (comment), I agree we don't necessarily want to have the app hosting the SLA itself to support deployments without omero-signup but with their own SLA e.g. in our case, outreach, nightshade, learning, idr.

An optional checkbox with an hyperlink to a static page would probably cover most of our use cases.
