improper appeareance of checkboxes in file types

What steps will reproduce the problem?
1. select from Type window-select Radio button of Book
2. validates and get file type window- point the arrow at right side of
check box of .pdf 
3.

What is the expected output? What do you see instead?
besides to(left side of) this check box .rtf this check box is not
appearing properly .

What version of the product are you using? On what operating system?
windowsXp

Please provide any additional information below.
if u point the arrow to any one right side of check box you will find same
improper appearence of check boxes of left side respectively.
