fortify2hdf not populating file and function names (and more)

The resulting HDF file from the fortify2hdf module is not replacing the function or file names. Please see the screenshot in which I've highlighted where the function and file name should be:

The same is true for every element in an FVDL file that is described in the <ReplacementDefinitions> block:

These items aren't being replaced/populated in the HDF file, so we can't tell what the source of a finding is.
