Add comments button does not appear on all lines

Steps to Reproduce:

1. Checkout branch with pull request.
2. I can only add a comment on certain lines. On other lines, the add comment symbol does not pop up.

It appears that \begin{ans} and \begin{enumerate} prevents the extension from showing add comment button.

The GitHub Pull Request extension lets you add comments on the same lines that GitHub.com does. This is all GitHub supports.
