- Passengers who in the past 14 days have been in, or transited through, Botswana, Eswatini, Lesotho, Malawi, Mozambique, Namibia, South Africa or Zimbabwe, are not allowed to enter. This does not apply to:
- nationals of USA;
- passengers with a Permanent Resident/Resident Alien Card (Form I-551);
- spouses of nationals and of residents of the USA;
- children of nationals and of residents of the USA, or passengers with an IR-4 or IH-4 visa;
- parents/guardians of a national or a residents of the USA, who is unmarried and younger than 21 years;
- brothers/sisters, younger than 21 years, of a national or a resident of the USA, who is younger than 21 years;
- members of the U.S. Armed Forces, their spouses, and children;
- passengers with one of the following visas: A-1, A-2, C-2, C-3 (as a foreign government official or immediate family member of an official), E-1 (as an employee of TECRO or TECO or the employee's immediate family members), G-1, G-2, G-3, G-4, NATO-1 through NATO-4, or NATO-6 (or seeking to enter as a nonimmigrant in one of those NATO categories);
- passengers with a letter of invitation or other official document, issued by the USA, by other government or by the United Nations, traveling on duty.
- Passengers must have a COVID-19 vaccination certificate showing that they were fully vaccinated at least 15 days (day of vaccination + 14 days) before arrival. Passenger details (e.g., name and date of birth) in the certificate must match those stated in the passport/travel document.
- This does not apply to:
- passengers arriving from the USA
- nationals and residents of the USA;
- passengers younger than 18;
- passengers with a letter issued by a doctor proving that the passenger is medically unable to receive the vaccine;
- nationals of Afghanistan, Algeria, Angola, Armenia, Benin, Burkina Faso, Burundi, Cameroon, Central African Rep., Chad, Congo, Congo (Dem. Rep.), Cote d'Ivoire, Djibouti, Egypt, Ethiopia, Gabon, Gambia, Ghana, Guinea, Guinea-Bissau, Haiti, Iraq, Kenya, Kiribati, Liberia, Libya, Madagascar, Malawi, Mali, Mozambique, Myanmar, Namibia, Nicaragua, Niger, Nigeria, Papua New Guinea, Senegal, Sierra Leone, Solomon Isl., Somalia, South Sudan, Syria, Tanzania, Togo, Uganda, Vanuatu, Yemen and Zambia with a visa issued by the USA, excluding B -1 and B -2 visas;
- members of the U.S. Armed Forces, their spouses, and children younger than 18 years;
- merchant seamen with a C-1/D visa.
- Passengers must have a negative COVID-19 test taken at most 1 day before departure from the first embarkation point. Tests accepted are: antigen, NAAT, RT-LAMP, RT-PCR or TMA. Passengers details (e.g., name and date of birth) on the result must match those stated in the passport or other travel document. The test result must specify "negative" or "not detected". If marked "invalid" it is not accepted.
- This does not apply to:
- passengers arriving from the USA;
- passengers younger than 2 years;
- US military personnel;
- passengers with a positive COVID-19 test result issued at most 90 days before departure from the first embarkation point; and a letter issued by a health authority stating that the passenger has been cleared for travel. The test result must specify "positive" or "detected", if marked "invalid" it is not accepted.
- Passenger must complete an "attestation" which must be presented before boarding. The form can be found at https://www.cdc.gov/quarantine/order-safe-travel.html .
- Passengers are subject to medical screening and could be subject to quarantine.
- This does not apply to passengers with a COVID-19 vaccination certificate showing that they received:
- the second vaccine dose of AstraZeneca (Vaxzevria) at least 14 days before arrival;
- the second vaccine dose of Moderna (Spikevax) at least 14 days before arrival; or
- the second vaccine dose of Pfizer-BioNTech (Comirnaty) at least 14 days before arrival; or
- the single vaccine dose of Janssen at least 14 days before arrival. More details can be found at https://dphss.guam.gov/quarantine/
- Airline crew are subject to quarantine until their next flight.
- Nationals of the U.S.A. are allowed to enter with an expired passport.
- This does not apply to emergency passports which must be valid on arrival.
