Version number, release date, and library compatibility should be added to topmost comment

A version number and release date should be added to the topmost comment (of tableExport.js) so it is easy to determine the age of the file.

The comment should also list version requirements for each library the plugin is compatible with. For example, an old version of the jsPDF AutoTable plugin is needed as per #141.
