[BUG] re-authentication prompt doesn't show

Describe the Bug
After logging into Heroku and then changing my bearer token in the netrc file to trigger a re-authentication prompt, the dialog box just says an error has occured without prompting for re-authentication.

To Reproduce
Steps to reproduce the behavior:

1. Test plan @ token invalidation
2. After logging in, change bearer token in netrc file and wait for VSCode to update
3. See error message without re-authentication prompt

Expected Behavior
Re-authentication prompt should show with corrupted/invalid bearer token

Actual Behavior
Extension shows error popup, but no re-authentication prompt

Environment:

• OS: macOS 15.2
• VSCode v1.96.3
• Extension Version: v0.0.1
