Sometimes copilot ask for confirmation multiple times

Describe the bug

Sometimes copilot ask to edit a file and I choose "Approve all file operations in this session" I get the same message again (and maybe again and again) short after.

Steps to reproduce the behavior

I assume this happens when copilot wants to edit multiple files at once and I'm not fast enough to approve this, so there is something in the queue or similar that did not recognizes I al ready approved that.

Expected behavior

If I approve a file operation or command for the rest of the session I'm not asked again.

I'm also experiencing this issue. I select the option to, "Yes, and approve all file operations for the rest of the running session," and it will immediately ask me to approve a file operation again.
