Sign Up and Log In Failed

Describe the bug
when user trying to login or sign Up getting login and Sign up failed error.
Unable to register new user getting 'Sign Up failed' error
And
when user trying to login with google or twitter getting Login Failed Error.

To Reproduce
Steps to reproduce the behavior:

1. Go to http://badgeyay.com/
2. Click on Login Menu
3. Click on New to site? Create Account
4. Now fill the valid Sign Up details
5. Click on Sign Up Button
6. See error Displayed: Sign Up Failed ! Please try again

OR

1. Go to http://badgeyay.com/
2. Click on Login Menu
3. Click on "Login with Google" or "Login with Twitter" button
4. See error Displayed: Log In Failed ! Please try again

Expected behaviour

The user should be registered when they clicked on sign up button. And user should be logged in in when they clicked on 'Login with Google' or 'Login with Twitter' button.
