Print SCU layout Images are not rendering in the correct X/Y location

For example, printing 9 images on a 3x4 layout. See screenshot

Here is the same example rendering correctly if the 12 image boxes are in use

Also, the images are not in the correct order... looks like the image # 1 is always at the bottom right corner

I was able to duplicate this using the PrintSCP example in your git and Osirix as the PrintSCU.
