First tap on "Edit Questions" and fill out your questions one per line like so

Next tap on Save
Now tap on "New Review" and answer your preset questions
Use the Next button and at the end tap on Save
To see past reviews, edit and save past reviews, or to delete past reviews tap on "Past Reviews"
You don't have to redo the edit questions step every time.
Whenever the app saves a file a note will appear on the screen temporarily with the file location
