Duplicated entities on restart

I'm getting duplicated entities on restart. Some/all entities are being duplicated. Also some of the controls are changing to sensors in the process. Example below:

Device is online but the control entities are greyed out. Some of them can be seen at the top of the sensor section just below:

Also happening to sensors that duplicates are created:

Example of older entity that is a number (correct):

This is now a sensor with a different entity_id:

@elbowz any hints on where to look. I made the change in the PR I have open but not sure how to ensure that entities consistently get linked in same way?

I'm sorry @spinza, but I'm in vacancy until the end of August...and I totally forget this code base (it's been more than 1 year).

...my useless suggestion is to rollback to the previous HA version.

p.s. I don't use this HA extension anymore, and I think it should be totally rewritten exploiting the HA MQTT Discovery features
