Support distance matrix files with only a column or row header #104

Currently only the following is support for distance matrix file formats

ID	A	B
A	0	0.5
B	0.5	0

Should also support the following formats

A	0	0.5
B	0.5	0

A	B
0	0.5
0.5	0

Actually, we do already support

ID
A	0	0.5
B	0.5	0

But not without the ID header column name. I agree it could be useful to support this, as well as the version with only a header row.
