Use provider address for GMaps "Route to Provider" on RP data hovers

When clicking on "View on Google Maps" for the Route to Nearest Provider, the address of the provider is often slightly off from the address provided in Encompass.

I believe this is because we use the lat/long of the provider to calculate the distances. If we have a provider address we should use that when linking to a suggested route in Google Maps instead of the lat/long, which results in a different resulting address.
