Address level unique name constraint inadequate when the address is at top level

Check in staging db, for example.

select type_id, title, is_voided, parent_id, organisation_id from address_level where title = 'Harichandanpur (Block)';

We need custom constraint for this as well.

We could other places in the system where such issues exist due to null treatment of postgres unique constraints
