Describe the bug
Checkbox are not being reset after form submission

To Reproduce
Steps to reproduce the behavior:

Go to 'http://localhost:6044/checkbox-fields'
Check one of the checkboxes
Click on 'submit'

Expected behavior
Al checkbox should be reset
