C89 のサポートを切る #346

完全なサポート終了は breaking change となるため v5.0.0 で行う
しかし、現在でも C89 な C2A user は非常に限定的であるため、c2a-core 本体以外については v5.0.0 を待たずに C99 を前提とするように順次変えていく
