I can navigate all the way to play a video but whenever start playing the episode I find my self looking at the screen below.

Sorry I've never seen that problem myself, not sure what would cause it.
In netflixbmc settings, under advanced, do you have 'Use Chrome Control Plugin' enabled?

It only happens when trying to use profiles. Also when I start the app, the
profiles dialog comes up but it's empty. If I switch back to single user
the plugin it works fine. I really would like it to work with multiple
profiles though. We do have 3 profiles. Me, my wife, and the Netflix kids
profile. I'm also using ubuntu.

Is there anything special I need to do to make the profiles work?

I suspect this was fixed in Version 1.3.15. Prior to that the profile window would show no selections.
It may be working now.
