Split `Chunk::mem_offset` out of `Chunk::to_mem`, and use that to avoid passing a null pointer to `Chunk::to_mem`, which calls `ptr::offset`, which Miri diagnoses as Undefined Behavior. Fixes alexcrichton#19.
