Documentation is unclear about what type of app to create and how to chose user scopes

The UI shown on the front page of this repository seems to indicate two types of app that can be created, and it's not obvious or written which of the two should be picked.
The current Xero UI as I've experienced it has three options:

I chose custom connection but on the configuration screen the scopes offered are application scopes. At no point was I offered user scopes so have not been able to select openid as a scope for my app.

Worth nothing that the linked video tutorial was the most useful resource in getting this set up successfully. Details such as the application url and callback both having to be https://developer.xero.com/ made all the difference.
