Update the spec for headers to be compatible with RFC8941.

One thing to pay attention to as this proceeds is that the value in calls to "set a structured field value" should have a clear type. (whatwg/fetch#1672) Most of our current uses leave that type pretty implicit, which means another implementation might serialize things differently.
