chore: update User-Agent header placeholder and fernignore entries

Update the User-Agent header value to use the standard Fern version
placeholder and add new entries to .fernignore to exclude replay and
gitattributes files from Fern-managed tracking.

Key changes:
- Replace hardcoded version in User-Agent header with `0.0.0-fern-placeholder`
- Add `.fern/replay.lock`, `.fern/replay.yml`, and `.gitattributes` to `.fernignore`
