Add EAC Instruction boxes at page breaks

The EAC Ballot Design Guide, section 3.12, specifies three instructions to add below the last contest on a page. Our ballots should include two of these (see attached image)

• "Continue voting next page"
• "Thank you for voting!"

@trustthevote I noticed these when reviewing the EAC design document. I think these should be included in our ballots (only options 2 & 3, though). I also think that the final instruction should be "Thank you for voting!" and we should drop the "Please turn in your finished ballot" phrase.

Please let me know if you'd like me to include these design elements for our current milestone.
