Multiple attachments shown when we send a message from the App

Defect Description : If i want to send a message from the app then multiple attachments shown in the system but there is only one attachment linked with the respected
Steps To Reproduce :

1. Go to the Raven-dev.frappe.cloud
2. Go to the Employee
3. Click on the Send a Raven
4. Then in attachments you can able to see the multiple attachments but the employee has only one.

Actual behavior: It should not be multiple attachment
Expected behavior: It should be only one attachment
Device: Macbook
